ECG Challenge: Irregular Pulse During Cardiology Visit

A 58-year-old man with a history of hypertension (treated with amlodipine and hydrochlorothiazide) and coronary artery disease (treated with a beta-blocker and isosorbide mononitrate) visits his cardiologist for a routine physical examination. His pulse is noted to be irregular, and a 12-lead ECG is obtained.
