ECG Challenge: Unusual Heart Sound in a Woman With CV Risk Factors

A 57-year-old woman with a history of hypertension, diabetes, and hyperlipidemia presents to her primary care provider for a routine physical examination. She is not experiencing any cardiac symptoms. Her physical examination is unremarkable except for a widely split S2. Because she has risk factors for coronary artery disease, a routine ECG is obtained (Figure 1).
