Demonstration of the presence of monosodium urate crystals in synovial fluid or tophi aspirates is the gold standard for diagnosis and is recommended by the European League Against Rheumatism (EULAR) for investigation of any patient with an undiagnosed inflammatory arthritis. However, if synovial fluid analysis is not feasible, a clinical diagnosis is supported by the presence of classic clinical features, radiographic findings and exclusion of other possible diseases.[8]
