Obesity and AF have a strong causal relationship. Some epidemiological data suggest the existence of an obesity paradox for patients with AF. This notwithstanding, the phenomenon appears to be more likely due to incompletely understood pathophysiological mechanisms and to a series of factors and confounders that should be cautiously taken into account. In the comparison between NOACs and VKAs, underweight and healthy weight patients seem to gain the most favorable effects in terms of risk reduction for both stroke and major bleeding.
