Is there Biological Plausibility?

Evidence from experimental and diagnostic studies using functional cardiac imaging as a reference standard for exercise-induced myocardial ischaemia suggests a direct link between myocardial ischaemia and the secretion of BNP and NT-proBNP.[10–14] BNP and NT-proBNP concentrations were significantly higher in patients with adjudicated myocardial ischaemia as compared with patients without myocardial ischaemia.[10–14] However, the diagnostic accuracy of BNP and NT-proBNP concentrations for myocardial ischaemia was only modest, suggesting that myocardial ischaemia is only one of several triggers for BNP and NT-proBNP release. Considering that CABG is often associated with more complete revascularization compared with PCI, the presence of HF quantified by BNP and NT-proBNP concentrations may identify patients who will particularly benefit from complete and long-lasting revascularization provided by CABG.
