ECG Challenge: Respiratory Distress and a Rapid Rate

A 54-year-old man presents to the emergency department with exacerbation of chronic obstructive pulmonary disease. He is treated with intravenous steroids and inhalers. Although his respiratory status improved, he developed a rapid heart rate; an ECG was obtained.
