Neuronal hyperexcitability could trigger CSD. However, CSD may account for the pathophysiology in a fraction of patients experiencing MA. The notion that CSD may underlie the headache mechanisms in patients with MO is not well accepted. In such migraine patients, the migraine is characterized by alterations in synaptic transmission, altered neuronal excitability or specific dysfunction in neurotransmitters. Some alternative hypotheses include the serotonin pathogenesis, the cortical hypoexcitability/hyper-responsivity[36] and the reduced energy reserve hypothesis.[37,38] Nevertheless, the neurophysiological correlates of different migraines remain unclear.
