Monitoring of Patients With Glucocorticosteroid-induced Osteoporosis

Recommendations by the ACR emphasize that an important element of monitoring in patients on glucocorticoid therapy is serial BMD testing. However, these recommendations do not specify how long the intervals between successive tests should be. The DXA scans should be performed more frequently in patients taking high doses of steroids, who have concomitant risk factors of fractures, accelerated bone turnover and a previously observed decrease in BMD between prior tests. In addition, at each visit the patient should have assessed the risk of falls, height, medication compliance, and if required, the concentration of 25-hydroxyvitamin D3 and an x-ray of the spine.
