History of Present Illness
A 13-year-old girl presents to the nurse practitioner in the School Based Health Center with a complaint of "losing her hair" in patches on her scalp. The patient reports that she recently noticed two areas of hair on her scalp that were getting thinner. When she told her mother that her hair was coming out in the shower, her mother told her that it is normal to lose a certain amount of hair every day. The patient is worried that the hair loss might show and may not be normal. She reports that the areas are not painful and denies pulling or twisting her hair around her fingers.
