Abstract and Introduction


Problem Electronic prescribing with clinical decision support is widely accepted as a means to improve safety in the medication-use process. However, these early warning systems are not error-proof, as demonstrated by an event reported to ISMP Canada in which a patient's height and weight were transposed into an electronic chemotherapy prescribing system. ISMP Canada has encouraged us to share information about this event, which appeared in the July 31, 2010, issue of ISMP Canada Safety Bulletin,[1] with US readers to bring attention to this particular electronic prescribing system vulnerability.
