The 5-year survival rate of patients treated with radiotherapy is more than 90%.[41] Pure germinomas are considered to have a good prognosis and a low recurrence rate after total resolution. Germinomas usually invade locally into the ventricular system and then seed via the subarachnoid space; extradural spread may occur rarely with the invasion of adjacent structures like the petrous bone and carotid artery.[41] The presence of shunts provides a convenient route for dissemination into the CSF space. Abdominal metastasis via VP shunts and also hematogenous pulmonary metastasis and lymphatic metastasis are known to occur.[42,43] It is rare for a second germ cell tumor to occur at a different site and with different histological types after total resolution of a pure pineal germinoma.[1]
