Staging of Hepatocellular Carcinoma

Accurate staging for HCC is crucial not only for providing patients with prognostic information but also for deciding appropriate treatment options. Treatment decisions should be based on both the tumor burden and the degree of underlying liver dysfunction. Several systems have been proposed. The Barcelona Clinic Liver Cancer (BCLC) Staging System is unique in that it combines tumor burden, hepatic function, and performance status with an evidence-based treatment algorithm (Table 1; Fig. 1). Although there is not one universally accepted staging system, the BCLC classification appears to offer the most prognostic information[3,11] and has been endorsed by the AASLD.[7] The ability of the BCLC staging system to predict a significant difference in survival across different stages was recently validated in a large cohort of Taiwanese patients.[12] The BCLC is the only staging system validated in more than one country.
