Our findings suggest that scheduled methadone started 1-2 days before beginning fentanyl taper facilitates rapid fentanyl discontinuation and may prevent development of opioid abstinence syndrome in children who have received long-term infusions of fentanyl. Prospective studies are required to determine the optimal dosage of methadone, the fastest rate at which fentanyl may be discontinued, and the shortest duration of methadone treatment.
