Although diuretics and fluid restriction have been recommended for the treatment of symptomatic neonates, no rigorously collected data support this approach. In fact, a systematic review of furosemide use in preterm neonates with respiratory distress syndrome showed no long-term benefits and an increased risk of symptomatic patent ductus arteriosus (PDA). Infants with signs of failure may be treated initially with digoxin and diuretic therapy, but interruption of the ductus is required for definitive treatment.
