Answer

Patients with an NIH Stroke Scale score higher than 22 are considered to be at high risk for hemorrhagic conversion because of the probability of a large infarcted area. Patients with a score lower than 4 have only minor neurologic deficits, for which thrombolytic therapy is not indicated. On CT, high-risk patients often have early changes showing a large area of edema or mass effect.
