Only patients with acute massive PE (ie, those at the highest risk of immediate death) and those with submassive PE with RV strain (abnormal echo or biomarkers) are eligible for fibrinolytic therapy if no contraindications are present. Other types are treated with anticoagulants or antithrombotic therapy. [31]
