The benefits of fibrinolytic therapy are well established during the initial 12 hours after symptom onset. Guidelines mention that administration of a fibrinolytic agent should be considered in symptomatic patients presenting more than 12-24 hours after symptom onset with STEMI affecting a large area of myocardium or hemodynamic instability if PCI is not available.
