Evaluation of febrile neonates for herpes simplex infection should be considered when the following risk factors are present:

  • Age younger than 3 weeks

  • Vesicles

  • Seizure

  • Toxic or ill appearance

  • CSF pleocytosis or red blood cells

  • Maternal history of herpes
