Caution with bicarbonate therapy is indicated because of its potential complications, including the following:

  • Volume overload

  • Hypokalemia

  • CNS acidosis

  • Hypercapnia

  • Tissue hypoxia via leftward shift of hemoglobin-oxygen dissociation curve

  • Alkali stimulation of organic acidosis (lactate)

  • Overshoot alkalosis
