Epsilon-aminocaproic acid (epsilon-ACA) has been used intraoperatively to inhibit fibrinolysis. It has been found effective for decreasing blood requirements in some studies but not in others. In a prospective, double-blinded, placebo-controlled, randomized study by Dalmau et al, prophylactic epsilon-ACA did not reduce intraoperative total PRBC transfusion during OLT; however, tranexamic acid did significantly reduce intraoperative transfusion requirements.
