After OLTX, the biliary tree of the allograft is solely dependent on hepatic artery perfusion to maintain its viability. Disruption of hepatic artery flow results in biliary ischemia or necrosis, with subsequent biliary strictures, bile leaks, bilomas, infected bilomas, and frank intrahepatic abscesses. [24] Hepatic infarction, fulminant hepatic failure, bacteremia, and sepsis can also occur. Complications involving the hepatic artery include hepatic artery thrombosis (HAT), hepatic artery stenosis (HAS), pseudoaneurysm, and arteriovenous fistula. [25]
