Adverse effects of pegloticase include anaphylaxis, infusion reactions, gout flares, and exacerbation of congestive heart failure. At present, substantial expense compromises its cost-effectiveness as an initial approach. [180] The ACR guidelines do not recommend pegloticase as a first-line approach.
