  • High levels of distress

  • Long-standing fibromyalgia

  • Major psychiatric disease or severe depression and anxiety that responds poorly to treatment

  • An ingrained pattern of work avoidance

  • Marked functional impairment despite multidisciplinary approaches to treatment

  • Opioid or alcohol dependence
