Ideally, all patients who have suffered a vertebrobasilar stroke should be admitted to a unit specializing in the care of stroke patients. Admission to a neurologic intensive care unit (ICU) is indicated for patients who are candidates for interventional therapies (eg, thrombolysis) or who have any of the following [28] :

  • Unstable or fluctuating neurologic symptoms

  • Decreased level of consciousness

  • Hemodynamic instability

  • Active cardiac or respiratory problems
