Mok et al conducted a large phase III study that compared gefitinib with carboplatin-paclitaxel in the first-line setting in Asian patients who had adenocarcinoma and had never smoked or were former light smokers (none in last 15 years). Patients receiving gefitinib had a higher response rate (43% vs 32%), with similar median survival (18.6 vs 17.3 mo). Of patients who were positive for the EGFR gene mutation, those in the gefitinib group had significantly longer progression-free survival than those in the carboplatin-paclitaxel group. Conversely, patients who were negative for the mutation had significantly longer progression-free survival with carboplatin-paclitaxel. [154]
