Adults with SCD should be evaluated for known stroke risk factors and managed according to the 2014 AHA/ASA primary stroke prevention guidelines. [45]

The AHA and ASA also provided guidelines for the prevention of stroke in patients with stroke or transient ischemic attack. These secondary stroke prevention guidelines include recommendations for controlling risk factors and the use of antiplatelet agents. Other therapies to consider in preventing recurrent cerebral ischemic attacks in adults with SCD include regular blood transfusions (to reduce HbS to < 30%-50% total hemoglobin), hydroxyurea, or bypass surgery for advanced occlusive disease.
