NCCN guidelines recommend the following first line treatment regimens (in order of preference) [20] :

  • DA-EPOCH-R: dose-adjusted etoposide, prednisone, vincristine cyclophosphamide, doxorubicin and rituximab (6 cycles); radiation therapy can be added for persistent focal disease
  • RCHOP + RT: rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (6 cycles) plus radiation therapy 
  • RCHOP + ICE with/without RT: rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (4 cycles) followed by ifosfamide, carboplatin, etoposide (3 cycles) with or without radiation therapy
