Answer

Obtaining a urine culture is necessary to direct appropriate treatment following intiial empiric therapy. In the setting of suspected pyelonephritis or flank pain with presumed infection, obtaining renal imaging is warranted to rule out obstruction and infection of the upper urinary tract, which requires prompt urologic consultation and intervention.
