What 30-day postoperative mortality rates are associated with Model for End-Stage Liver Disease (MELD) scores in patients with cirrhosis?

In a study of patients with cirrhosis who underwent major digestive, orthopedic, or cardiovascular surgery, the preoperative MELD scores and their associated 30-day postoperative mortality rates were as follows [69] :

  • MELD score of 7 or less - 5.7% mortality

  • MELD score of 8-11 - 10.3% mortality

  • MELD score of 12-15 - 25.4% mortality

  • MELD score of 16-20 - 44% mortality

  • MELD score of 21-25 - 53.8% mortality

  • MELD score of greater than 26 - 90% mortality

The benefits and the risks of surgery should be carefully weighed before advising the patient with cirrhosis to undergo surgery.
