Aggressive therapy should be considered early in the management of patients with difficult or refractory inflammatory bowel disease (IBD). This approach uses immune modifiers and/or anti-tumor necrosis factor (TNF) agents earlier in the treatment of the IBD patient (see Overview of Therapy).
