Patients with moderate to severe Crohn disease who have documented active inflammation, dependence on corticosteroids and an inability to taper these agents, or disease refractory to steroids are most likely to benefit from anti-TNF therapy. [96] Before anti-TNF agents are administered, screening should be done for coexistent infection with perianal and abdominal abscess (including Mycobacterium tuberculosis), and caution is advised if a patient is a carrier for the hepatitis B virus.
