Bivalirudin is direct thrombin inhibitor that is given as 0.1 mg/kg loading dose, followed by 0.25 mg/kg per hour only in patients managed with an early invasive strategy. This regimen is continued until diagnostic angiography or PCI. [107] In the Acute Catheterization and Urgent Intervention Triage Strategy (ACUITY) Trial, the efficacy of bivalirudin was noninferior to the use of heparin plus GP IIb/IIIa. [108]
