The most feared complication of the treatment of PE is severe and fatal bleeding. Major risk factors for bleeding include intensity and duration of therapy, increased age, and significant hepatic or renal dysfunction. Comparison studies of the incidence of severe and fatal bleeding complications between heparin and rt-PA have demonstrated no significant differences. When significant bleeding does occur, it may be necessary to treat with agent-specific strategies. [59]
