The 2013 guidelines for early management of acute ischemic stroke recommend that although in most instances CT provides the necessary information, MRI may be used before intravenous TPA administration to exclude ICH (absolute contraindication) and to determine whether MRI hyperintensity of ischemia is present (Class I; Level of Evidence A). [16]
