Use and abuse of legal and illegal drugs have also been associated, probably causally, with ischemic stroke and intracerebral hemorrhage. The drugs most specifically related to strokes include the following:

  • Ephedrine

  • Pseudoephedrine

  • Phenylpropanolamine

  • Heroin

  • Cocaine

  • Amphetamines

  • Alcohol

These drugs may cause stroke by means of several mechanisms (eg, hypertensive surges, arterial vasospasm, arrhythmias, hypercoagulopathy, and embolization of diluent particles). Treatment should be focused on stopping use of the drug and on rehabilitation. [72]
