What is the role of cerebral atrophy in the pathogenesis of chronic subdural hematoma (SDH)?

Given that the pathophysiology of chronic subdural hematoma is often directly associated with cerebral atrophy, the fact that subdural hematomas are associated with conditions that cause cerebral atrophy (eg, alcoholism, dementia) is not surprising. In a series reported by Foelholm and Waltimo, alcoholics constituted over half of the patient population. Most chronic subdural hematomas are probably caused by head injury; other causes and predisposing factors include coagulopathy, use of anticoagulants (including aspirin), seizure disorders, and CSF shunts.
