The Patient-Centered Medical Home (PCMH) is a multi-organizational collaborative whose goal is to provide comprehensive primary care for children, youths, and adults. The PCMH model is a health care setting that facilitates partnerships between individual patients and their personal physicians. It strengthens these relationships by replacing episodic care based on illnesses and patient complaints with coordinated care and a long-term healing relationship. A care team is led in most--but not all -- cases by a physician, who provides all the patient's health care needs and, when necessary, arranges for appropriate care with other qualified health-care providers. Enhanced care is also provided through open scheduling, expanded hours and communication between patients, physicians, and staff.
