Table 1. Values of Liver Function Tests Before and After Treatment with Infliximab

Test Serum Concentrationa
Before Infliximab After Infliximab
Patient 1
   Alanine transaminase (units/L) 25 45
   Aspartate transaminase (units/L) 28 47
   Alkaline phosphatase (units/L) 159 146
   Lactate dehydrogenase (units/L)b 465 412
   Total bilirubin (mg/dL) 0.10 0.12
   Direct bilirubin (mg/dL) 0.05 0.04
Patient 2
   Alanine transaminase (units/L) 49 58
   Aspartate transaminase (units/L) 36 39
   gamma-glutamyl transferase (units/L) . . . 665
   Alkaline phosphatase (units/L)c 357 428
   Total bilirubin (mg/dL) 0.34 0.16
   Direct bilirubin (mg/dL) 0.11 0.33
Patient 3
   Alanine transaminase (units/L) 165 56
   Aspartate transaminase (units/L) 65 18
   Lactate dehydrogenase (units/L) 371 745
   Total bilirubin (mg/dL) 4.63 1.67
   Direct bilirubin (mg/dL) 3.38 0.55
Patient 4
   Alanine transaminase (units/L) 35 62
   Aspartate transaminase (units/L) 20 62
   Alkaline phosphatase (units/L) 83 100
   Total bilirubin (mg/dL) 0.21 0.40
   Direct bilirubin (mg/dL) 0.06 . . .

aNormal values: alanine transaminase, 0-35 units/ L; aspartate transaminase, 0-35 units/L; alkaline phosphatase, 30-114 units/L; lactate dehydrogenase, 118-242 units/L; total bilirubin, <1.2 mg/dL; direct bilirubin, <0.2 mg/dL; gamma-glutamyl transferase, 0-33 units/ L (females) or 11-51 units/L (males).
bElevated lactate dehydrogenase concentrations are neither sensitive nor specific indicators of liver damage. Since the results of other tests in patient 1 were normal or only mildly elevated, the elevated lactate dehydrogenase concentrations were judged not to indicate liver involvement.
cBecause of the concurrently elevated gamma-glutamyl transferase concentration, the elevated alkaline phosphatase concentrations were judged to indicate liver involvement.
